Prosthetic valve dysfunction Changes in symptoms or physical findings are usually the first signs of prosthetic valve dysfunction. Deterioration of clinical status should always be taken as prosthetic valve dysfunction unless proved otherwise. Auscultatory findings suggestive of prosthetic valve dysfunction Findings common to any position of prosthetic valve: Absence or definite reduction of a prosthetic opening click (OC) or closing click (CC) as previously heard either by doctor or patient Change in intensity of OC to CC clicks Beat to beat variability in the intensity of OC or CC clicks or random alterations in the timing relationship of prosthetic sounds Findings with prosthesis in mitral position. Presence of PSM (pansystolic murmur) at apex A prominent MDM at apex or changing or ‘new’ MDM ≤ 2 grade murmur may be a normal finding especially in Bileaflet or bioprosthetic valve Either prolonged S2-OS (opening sound) or shortened S2-OS interval Prolonged if Interference with ball excursion Shortened if Prosthetic obstruction (< 0.1="" msec)="" severe="" paravalvular="" regurgitation="" findings="" with="" prosthesis="" in="" aortic="" position="" presence="" of="" long,="" ‘kite’="" shaped="" esm:=""> grade 2 murmur Presence of EDM (early diastolic murmur) A faint diastolic murmur may be present in tilting disc valve * Significant prosthetic dysfunction may be present even without audible changes in valve opening or closing sounds or without pathognomonic

About the Author
The author is an interventional cardiologist. He has completed his cardiology training from prestigious G S Seth Medical college and KEM Hospital, Mumbai, India. He has worked as cardiology fellow in Glasgow Royal Infirmary, Glasgow for 1 year. After completion of fellowship he has been practicing cardiology in Gujarat, India for last ten years. He has vast experience in Coronary as well as Rheumatic heart disease (RHD). RHD is still quite common in India and is a major cause of admission in hospitals. The author has rich experience of balloon valvuloplasties. His experience in dealing with valvular heart disease gives him an edge in writing a detailed book about multivalvular and prosthetic valve disease. In his series of handbooks about valve disease, he has already published "A Handbook of Rheumatic Fever", "A Handbook of Aortic valve disease and "A Handbook of Tricuspid and Pulmonary Valve disease". All these books have been published by Authorhouse. Apart from this handbook he has been a co-author of a book called "Patel's Atlas on Transradial Interventions: The basics" and has contributed as an author in "Patel's atlas on Transradial Interventions: Basics and Beyond".
